Abilities and Possibilities I Program

Service

Abilities and Possibilities I is a skills based program for persons with developmental disabilities and other related impairments. The program will focus on five areas: Health and Wellness, Community Safety, Functional/Academic Skill Building, Cultural Diversity and Leisure/Social Activities.  The program is designed to have sessions that include breaks and transition times. The individuals will participate in community events and outings, guest lectures, civic groups and community forums. The population served is in need of life skill training and adaptations to barriers that inhibit community integration. The program is designed to give individuals with disabilities an opportunity to participate in community and site based activities. The program will assist the community by providing education to its citizens and support to family members of individuals with various disabilities.

Case Management Services, Physiological Evaluation or Comprehensive Clinical Assessment, Natural Supports or Family Involvement

Population

The population served will be adults with developmental disabilities, mental health diagnosis', physical disabilities and other impairments in Mecklenburg County and the surrounding areas. This program is for the individuals that may need to work on skill set needed for employment or steady volunteer opportunities.

Setting

The program will be based out of Nevins Inc. Charlotte campus and the community at large. Individuals will participate in a daily community event.

Staff

The program will be managed by a program director/qualified professional. The daily activities and goals will be implemented by the APP coaches. Staff Ratio: 1 to 5

Operations

The Abilities and Possibilities I Program operates Monday through Friday from 8:00 a.m. until 2:00 pm (6 hrs per day).  It will be $250 per month.

Transportation

Program participants will have to provide their own transportation using personal vehicles, STS or county transportation. Nevins Inc. will assist in coordination of the needed transportation services.

Sharing of Information

Program information will be shared with Area Mental Health (LME), Department of Health and Human Services, case management and service providers.

Information about the participant will not be shared outside of the agency unless specific written consent is provided. It is the responsibility of the participant and caregiver to name in writing individuals, groups, and organizations to which information is to be shared or not shared.
