Service
Abilities and Possibilities II is a program for persons with developmental disabilities and other related impairments. The population served is in need of additional supervision and structured activities during the day. The program provides age appropriate educational, social, recreational and stimulating activities in a safe and supportive environment. The program is designed to have sessions that include breaks and transition times. The sessions may include current events, warm-ups, arts and crafts, functional skill building, leisure/recreational activities, social activities and outdoor activities. The program is designed to give individuals with disabilities an opportunity to participate in community events, . The program will assist the community by providing education to its citizens and support to family members of individuals with various disabilities.
Prerequisites
Case Management Services, Physiological Evaluation or Comprehensive Clinical Assessment, Natural Supports or Family Involvement

Population
The population served will be adults with developmental disabilities, mental health diagnosis', physical disabilities and other impairments in Mecklenburg County and the surrounding areas.
Setting
The program will based out of Nevins Inc. Charlotte campus and the individuals will also participate in community outings.
Staff
The program will be managed by a program director/qualified professional. The daily activities and goals will be implemented by the A&P coaches.
Operations
The Abilities and Possibilities program operates Monday through Friday from 8:00 a.m. until 2:00 pm (6 hrs per day). This is $150.00 per month.
Transportation
Program participants will have to provide their own transportation using personal vehicles, STS or county transportation. Nevins Inc. will assist in coordination of the needed transportation services.

Sharing of Information
Program information will be shared with Area Mental Health (LME), Department of Health and Human Services, case management and service providers.
Information about the participant will not be shared outside of the agency unless specific written consent is provided. It is the responsibility of the participant and caregiver to name in writing individuals, groups, and organizations to which information is to be shared or not shared.
