Service
Community Assessment and Educational Pathways Program also known as CAEPP is a community program for persons with developmental disabilities and other related impairments. The programs will asses the skills, abilities and level of care needed for individuals to fully integrate into the community. The pathways are vocational, volunteer, life skills, activity/functional skill building or activity with medical assistance. The individuals will work one on one with the CAEPP coaches in this program. The program is broken up into three intervals: During the first 30 days (initial assessment period) the individual will be exposed to the four pathway areas to identify their areas of interest, strengths and weaknesses. During the intermediate phase (30 to60 days) the areas of interest, strengths and weaknesses that were identified will be explored in greater detail. This will allow coaches and individuals to pinpoint the exact pathway they should explore. The third phase (graduation 60 to 90 days) this will be the transition period to the pathway the individual had chosen. In this phase program directors, coaches, parents/guardians and the individual will meet to discuss the pathway that has been chosen by skill level and individual interest. The program then will work on any barriers to the pathway choice that would possibly inhibit them from participating fully. After these issues have been addressed or identified, there will be a CAEPP graduation to symbolize the completion of the program and the beginning of a new journey in their new pathway. The program will assist the community by providing education and support to families/guardians of individuals with developmental disabilities and various impairments.

Development Disabilities or Mental Health Diagnosis’, Psychological Evaluation, Person Centered Plan, Case Management
Population
The population served will be adults with developmental disabilities, mental health diagnosis', physical disabilities and other impairments in Mecklenburg County and the surrounding areas.
Setting
The program will be based out of Nevins Inc. Charlotte campus and the community at large.
Staff
The program will be managed by a program director/qualified professional. The daily activities and goals will be implemented by the CAEPP coaches.
Staff Ratio: 1 to 1
Operations
The Community Assessment and Educational Pathways Program operates daily, times are negotiated between individuals, families and coaches.

Program participants will be transported by CAEPP coaches.

Program information will be shared with Area Mental Health (LME), Department of Health and Human Services, case management, the community and service providers.
Information about the participant will not be shared outside of the agency unless specific written consent is provided. It is the responsibility of the participant and caregiver to name in writing individuals, groups, and organizations to which information is to be shared or not shared.
