Provider Organization Requirements
Nevins Inc. is a qualified provider organization that meets the standards set forth by MH/DD/SAS. Nevins Inc. is a CARF accredited non-profit organization that is legally recognized to do business in the United States and North Carolina. Nevins Inc. employs a full-time Executive Director (Kathy M. Jackson, MS). Nevins Inc. maintains supportive, therapeutic relationships between our agency and the individuals we serve. This relationship addresses and/or implements interventions outlined in the person-centered plan. Nevins Inc. has implemented the philosophy and principles of self- determination, person centered thinking and person centered planning. Consumer choice and consumer evaluations are an integral part of the services our agency provides.
Staffing Requirements by Age/Disability
Nevins Inc. CAET services will be under the direction of a Qualified Professional (QP) status according to 10A NCAC 27G.0104. The CAET- QP is responsible for the supervision of program staff which may include other Qualified Professionals (QP), Associate Professionals (AP) and paraprofessionals (PP) who meet the requirements according to 10A NCAC 27G.0104 and who have the knowledge, skills and abilities required by the population to be served/supported.
The CAET- QP will be responsible for updating and monitoring the medical records, preparation and implementation of Person Centered Plans, monitoring and analyzing monthly data reports, completing Quarterly Reviews, staff training and natural support meetings/workshops.* The CAET- QP will link, monitor and coordinate services of the participants in the CAET Service.
The staff ratio is dependent on the mix of persons being supported at any given, and the mix of staff that form a person’s “life, community, support team” if one is needed. Staff will not support more than 5 individuals at any one time.
CAET support coaches are required to participate “face to face” in every person centered plan session , i.e. , in the initial development of the plan and all subsequent sessions specific to plan modifications and/or revisions.
* CAET support coaches will be required to participate in the monthly natural support/CAET meetings for the specific individuals on their caseload.
Program and Staff Supervision Requirements
The goals and objectives of CAET will be implemented based on the needs of the program participant. These needs will be showcased in the Person Centered Plan that will be created in collaboration with the individual, their natural supports and CAET-QP. The authorization units will be specified in the individuals Person Centered Plan. The service is available for one or more hours per day. Services may be provided on weekends or in the evenings. The normal day will be between three to six hours per day per individual. Staff are considered “life integration coaches” and are supervised by a Qualified Professional who has broad knowledge of self-determination, self-advocacy, individual rights, person centered planning and person first language.
Requisite Staff Training
Staff will have received training and are knowledgeable in field of intellectual and cognitive disabilities, physical disabilities, mental health diagnosis’, person centered planning, person first language, self-determination, self-advocacy, NCI Training, First Aid, CPR, Seizure Training, Medication Training and Traumatic Brain Injury.
Service Type/Setting
Services are community based and all goals and objectives will be achieved outside of a segregated setting. The individuals will participate in activities where they live, work and socialize.
Program Requirements
The Nevins Inc. CAET program is an individualized, one on one service, with one staff member delivering services to one individual for the authorized service units one hundred percent in the community.
The staff ratio will depend on the level of support that is needed for each individual at any given time.
The maximum caseload of a CAET “life integration” coach will be 4- 5 individuals at any given time.
The CAET participants may work with other individuals in training or peer support meetings.
The CAET “life integration” coach will meet with the program participants face to face and as much or as little is needed to accomplish the goals and objectives set forth in the person centered plan.
Natural Supports such as parents, guardians, AFL providers, and friends will have to participate in monthly CAET support meetings and natural support training workshops.
Entrance Criteria
Per 10 NCAC 14V .2306 (b) (3), a qualified professional or an associate professional shall certify the eligibility of each individual for the CAET service according to the following criteria:
A. There is an Axis I or Axis II diagnosis of a developmental disability as defined in GS 122C-3 (12a) or the person may have a diagnosis of developmental disability and a co-occurring diagnosis of mental illness.
AND
B. Level of care Criteria, NC SNAP
AND
C. The individual is experiencing difficulties in at least one of the following areas:
- functional impairment
- crisis intervention/diversion/aftercare needs, and/or
- at risk of placement in a more restrictive setting
AND
D. The individual’s level of functioning has not been restored or improved and may indicate a need for intensive supports in a natural setting if any of the following apply:
- At risk for out of home placement , hospitalization, and or institutionalization due to symptoms associated with diagnosis.
- At risk of exclusion from services, placement or significant community support systems as a result of functional or behavioral issues associated with the diagnosis.
This service will be available to all individuals with a disability including those with complex and significant disabilities and a” level of supports system” will measure the extent and duration of support an individual needs. A”level of support” system is defined as:
Intermittent support (episodic need)
Limited support (needed for specific periods of time)
Extensive support (needed regularly for an extended period of time, and
Pervasive support (life long, intense need).
Entrance Process
- Person centered plan created by CAET-QP that focuses on hopes, dreams, community, work and life integration.
- Authorization by the Local Management Entity is required
- CAET service must be included in an individual’s person centered plan prior to or on the date services are to begin.
- Initial authorization for services will not exceed ninety (90) days. Re- Authorization will be quarterly.

Continued Stay Criteria
The desired outcomes or level of functioning have not improved or may not continue over the time frame outlined in the person centered plan without the CAET service. If outcomes have not been met, the goals and objectives the person centered plan will be reevaluated.
Transition Criteria
- Individual requests transition from program
- Individual is not achieving stated outcomes from the CAET service and an alternate service is identified
- Individual has achieved expected outcomes
- The individual can sustain outcomes without the service
Long Term Follow Along
After the Individual has completed the CAET program or has been transitioned for other reasons, The CAET “life integration” coach will work with the individuals and their families or guardians for as long as there is a need. The CAET “ life integration” coach will assist with advocacy, time management, employer relations, and any unforeseen issues that may go on in a person’s daily life. The CAET “life integration” coach will link, coordinate and monitor their services. The CAET “life integration” coach will see each individual at a minimum three hours per month.
Evaluation of Consumer Outcomes and Perception of Care
The expected outcome is that the program participants will increase their level of personal independence and learn to depend more on themselves and their abilities using self-determination and self advocacy skills. The supports will be based on person centered planning, best practices and using a holistic approach to independence.
