Service
Senior Supremes is a day health program for persons with developmental disabilities and other related impairments that may need extra assistance with toileting, medication and food consumption. The program is not limiting to assisting in those areas. The population served is in need of additional supervision and structured activities during the day. The program provides age appropriate educational, social, recreational and stimulating activities in a safe and supportive environment. The program is designed to have activity sessions that include breaks and transition times. The activity sessions may include current events, warm-ups, arts and crafts, functional skill building, leisure/recreational activities, social activities and outdoor activities. The program is designed to give individuals with disabilities and health related issues the opportunity to participate in activities in a group setting. The program will assist the community by providing education and support to family members of individuals with various impairments.
Population
The population served will be adults with developmental disabilities, mental health diagnosis', physical disabilities, health related issues and other impairments in Mecklenburg County and the surrounding areas.
Setting
The program will be based out of Nevins Inc. Charlotte campus and the community at large.
Staff
The program will be managed by a program director/qualified professional. The daily activities and goals will be implemented by the activity coaches and CNA.

Operations
Program participants will have to provide their own transportation using personal vehicles, STS or county transportation. Nevins Inc. will assist in coordination of the needed transportation services.

Fees
The fee for this service is $400 per month. The fees are paid privately by the individual or guardian. Payment is expected on the first day of the month.

Sharing of Information
Program information will be shared with Area Mental Health (LME), Department of Health and Human Services, case management and service providers.
Information about the participant will not be shared outside of the agency unless specific written consent is provided. It is the responsibility of the participant and caregiver to name in writing individuals, groups, and organizations to which information is to be shared or not shared.
