Service
The Transitioning Teens Afterschool Program (TTAP) will be an extension of the normal school day by providing a safe, structured and enriching environment for individuals between the ages of 16 and 22 with development disabilities or other impairments. The program will focus on communication skills, self-advocacy and management skills, navigating the community, health and safety education, social and leisure activities, life and employment skills after normal school hours. The program will provide exposure to various employment and volunteer opportunities through community based assessments, tours, field trips, guest speakers and educational forums. The program is designed into three learning sessions that will include breaks and transition times. The Transitioning Teen afterschool program will give young individuals a head start on the next phase of their vocational and educational journey after the completion of high school. The program will assist the community by providing afterschool care, respite and support to family members of individuals with various impairments.

Population
The population served will be students and individuals ages 16 to 22 with developmental disabilities, mental health diagnosis', physical disabilities and other impairments in Mecklenburg County and the surrounding areas.
Setting
The program will be based out of Nevins Inc. Charlotte campus and the community at large.
Staff
The program will be managed by a program director/qualified professional. The daily activities and goals will be implemented by the transitioning teen coaches.
Operations
The Transitioning Teen program operates Monday through Friday from 2:00 pm until 6:00 pm (4 hrs per day).
Transportation
Program participants will have to provide their own transportation using personal vehicles, STS or county transportation. Nevins Inc. will assist in coordination of the needed transportation services.
Fees
The fee for this service is $400 per month. The fees are paid privately by the individual or guardian. Payment is expected on the first day of the month.

Sharing of Information
Program information will be shared with Area Mental Health (LME), Department of Health and Human Services, case management and service providers.
Information about the participant will not be shared outside of the agency unless specific written consent is provided. It is the responsibility of the participant and caregiver to name in writing individuals, groups, and organizations to which information is to be shared or not shared.
